Job Description

Roles and Responsibilities

  • Preparing In-patient Discharge Summaries:
    • Updating details from antenatal card & case-sheet into the software: i.e. ANC info, Admission details, Normal delivery, Caesarean Info, Hospitalization after delivery, Investigations, Discharge info & Summary Info.
    • Types of Summaries: Antenatal / Abortion or miscarriage / Normal delivery / Caesarean delivery / Post-Natal observation / General observation / Chemotherapy cases. Summaries to be kept ready before discharge.
    • Fair copies: In coordination with the concern doctors rough copy summaries are made into fair copies and handed over to ward-coordinators.
    • Letters to referral doctors (if any) are prepared at the time of discharge and are submitted to HR Dept. for posting.
  • Preparing Medical certificates / Out-Patient summaries:
    • Medical certificates / Out-Patient summaries: After approval of the consultant out-patient summaries & medical certificates are made in coordination with Registrar or consultant (Rough copy followed by fair copy).
    • Types of certificate: EDD certificate / Delivery certificate / Maternity leave certificate / Paternity leave certificate / Travel fitness certificate / Emergency certificate / Certificate for restricted activity / Second copy case summaries etc.
  • Scanning & Emailing: Sending medical certificates / Out-patient & In-Patient summaries : through emails to the patients on their request.
